Teen Assessment

This assessment is for a teen who is 17 and ready to work at the adult level. The assessment requires a guardian to be present to discuss the adult nature of the classes and material used.

Waiver of Liability:
As a participant in any program or class at Arizona Actors Academy/Theatre Upstairs (Hotchner Studios LLC), understand and agree that in participating in any acting/voice/movement class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I recognize and acknowledge there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damages or loss that I or my minor child may sustain as a result of participating in any activities connected with or associated with such program or class. I agree to waive and relinquish all claims I or my minor child may have as a result of participating in any program or class against Arizona Actors Academy/Theatre Upstairs. (Hotchner Studios LLC) and its officers, agents, servants, employees and independent contractors. I further agree to indemnify and hold harmless and defend Arizona Actors Academy/Theatre Upstairs (Hotchner Studios LLC) and it's officers, agents, servants, employees an independent contractors from my claims resulting from injuries including death, damages and losses sustained by me or my minor child that arise out of, in connection with, or in any way associated with the activities of this program.
• TERMS: Private coaching is based on availability. After your request for Private Coaching you will receive contact from your coach to discuss or confirm scheduling.
• PAYMENT: Your information will be charged immediately for your selected coaching upon submission of this form. This payment will process through stripe. We do not keep your information.
• CANCELLATION & REFUNDS: 24-hour cancellation notice is required. Every effort will be made to reschedule your private session as long as the instructor receives 24-hour notice. There are no refunds for missed appointments. Private coaches are separate entities from AZAA programming therefore any services paid is non-transferable to AZAA tuition costs.
• INCLEMENT WEATHER OR INSTRUCTOR EMERGENCY: In the event of inclement weather or emergency, we will send email notifications if your session is cancelled. Any cancelled sessions will be offered a make-up date with ample time to adjust your schedule.
• COACHING RATES: Rates are subject to change.

COVID 19 Update

We do not require masks and leave it up to the individual to decide what is best for them. Our coaches are vaccinated and will not be wearing masks. By attending our events, you assume the risk of covid exposure and will take your own measures. Do not attend your session if you are sick. *
